EMERGENCIES / Messages​

Call 911 or visit an emergency center if you are experiencing a life-threatening emergency. 

Call 988 for the Crisis Lifeline a 24/7, free and confidential support line for people in distress as well as for prevention and crisis resources for you or your loved ones.

For clinical issues or concerns, you may reach ABH through your patient portal. 

For other questions/matters, you can leave a voicemail message at 904-274-1080.

Follow up Visits

It is important to have a plan in place to track progress, check symptoms as well as monitor and manage medications. Expect to follow up at least every 3 months. If you have not been followed up in 4 months, your chart will be closed and you will be scheduled as a new patient (evaluation) in order to re-open.

Medication Adherence

It is important that medications are taken as prescribed. If you are  unable to tolerate your medication, contact the clinic. ​

Prescription Policy 

Dr. Aguilar will provide prescriptions/refills that should be enough to last you until your follow up visit. If for some reason you are close to running out of your medication and will require a refill in between visits, please be reminded that refills will only be approved/called in if you have been seen in the last 3 months and you have an appointment on the books. 

Please call your pharmacy to ask for a refill at least one week before you run out of your medication.

Controlled Medications Policy

Controlled medications (stimulants, benzodiazepines, Z-drugs and tranquilizers) can be effective in the treatment of certain mental health disorders. As controlled substances are highly regulated, ABH follows a protocol when prescribing these medications.

Cancellation and No Show Fees 

To make sure we can help as many patients as possible, we charge a cancellation fee (less than 24 hours notice)/no show fee of $175 (New Patient Visit)/$90 (Follow up Visit). Please be mindful that if your appointment is on a Monday you should call to cancel by the preceding Friday. 

Medical Forms and Medical Records Fee

We do not charge a fee for most one page forms. There might be exceptions to this and you will be advised of the fee before the form is completed. Fees for other forms/records are listed below.

FMLA/Medical Leave - $50 

Narrative report - $100

Medical records - $1 per page up to 25 pages, and $0.25 cents per page thereafter

We will request payment at the time you submit the form/request to us. Please allow the clinic 10 business days to complete the form/request.
